This form can be completed if your child has a regular medicine such as an asthma inhaler or a medicated cream.

If your child is on medication on a short term basis then you will need to complete one of these forms each day that they take it. This includes Calpol. Please drop this form off with a member of staff in your room. Children must have been on medication for at least 24 hours before we can administer it.
